If you’re receiving treatment from one of our services and have questions about your care, please speak to the health professional who is looking after you. They will be able to help, advise and signpost you to additional resources if you need them.
Once your treatment is complete and we have discharged you from the service, you may be given self-care information by your health professional such as:
- an information leaflet
- online resources
- signposting to other services in community
- mobile health apps
If you feel you need further treatment from us, go back to see your GP for a referral or check to see if you can self-refer. Information about referrals is available on each individual service page.
What’s available on this website?
This website has been designed to help you find out about all the services we offer, where these services are available and how to access them. Below is a brief description of each section of the website to help you get around and find what you need.
Your health and care
This section of the website is designed to help you find health and care information quickly and easily. It will grow and improve over time as we add more self-care content to the site e.g. video content and listen to feedback from our services users about what they want to see.
All health and care services are listed here. You can search them by keyword or alphabetically. Find out what they do and how to access them.
As a community trust, we deliver services from over 50 locations in Wirral and Cheshire East communities. Use our locations search to find out where you need to go and how to get there.
This section is where you’ll find all the patient leaflets and information that each service has available. You can search by keyword and filter your results to find what you need.
Here you’ll find corporate information about the Trust including annual reports, board meetings and other statutory information.
