We provide a graduated offer of support for children and young people with Special Educational Needs and Disabilities (SEND) from birth to 19 years plus, working closely with families, schools, health services, and community partners. Our aim is to identify needs early, offer the right help at the right time, and ensure every child has the opportunity to thrive.
Early Years (0-5 years) – Health Visiting and Early Years SEND Support

We work with families from birth to help identify needs early and provide tailored support.
What we offer:
- Referrals to paediatricians, Speech and Language Therapy (SaLT) and other specialist services.
- SEND notifications and liaison with nurseries and early years settings.
- Named practitioner for children with identified needs.
- Annual reviews for children open to the service.
- Targeted 9 month and 2 year developmental checks at home for families who need additional support.
- A (proposed) dedicated pathway for extreme premature babies and those at risk of developmental delay, including home based reviews and Ages & Stages Questionnaire assessments at ages 3 and 4.
- Access to SEND advice appointments/drop‑ins and support through the 0-19+ Contact Hub and Local Authority SEND LIVE events.
- Course for parents and carers eg Understanding your Childs Development.
Primary School Age (5-11 years)

Supporting children as they start and progress through school.
What we offer:
- Universal School Entry Assessment for all children.
- Hearing and vision screening.
- Targeted follow‑up for families where concerns are identified.
- Signposting to SEND advice appointments/drop‑ins and specialist support and Local Authority SEND LIVE events.
- Brief interventions for:
- toileting and personal hygiene
- healthy eating
- sleep
- behaviour and emotional wellbeing
- Digital support including SEND webinars and parent courses.
- Support via the 0-19+ Contact Hub.
Secondary School Age (11-16 years)

Support for young people during adolescence.
What we offer:
- Access to ChatHealth for confidential advice.
- Targeted support from school nurses, SEND practitioners and mental health practitioners.
- Brief interventions for health, wellbeing, and behaviour.
- SEND advice appointments/drop‑ins at Family Hubs and Local Authority SEND LIVE events.
- Signposting to specialist services.
Post 16 (16-19+ years)

Support for young people as they move into adulthood.
What we offer:
- Support based on assessed need.
- Signposting to appropriate services, including health, education, and community support.
- Work underway to strengthen links with local colleges to support smooth transitions.
Home Educated Children

Support for families choosing to educate at home.
What we offer:
- Targeted support for children on Child in Need or Child Protection Plans or those children in care.
- Home assessments where needed, particularly where safeguarding concerns exist.
- Access to SEND advice appointments/drop‑ins, and Local Authority SEND LIVE events and 0-19+ Contact Hub support.
- Referrals to specialist services when required.
Specialist Schools

Support within specialist education settings including alternative provisions.
What we offer:
- Support tailored to autism‑specific and emotional health specialist settings.
- Signposting to additional services.
