Who are we?
Our Rehabilitation at Home Service is a team of occupational therapists, physiotherapists and technical instructors who support people who are having difficulty managing daily tasks at home due to recent changes in their health.
Our team provide support to enable people to rehabilitate and help improve their confidence and functional independence within their home environment.
Who is the service for?
The service is available for people who are:
- aged over 18
- registered with a Wirral GP
- having difficulty walking and/or managing daily activities at home
- looking to improve their independence with advice, equipment or exercise therapy
What do we do?
We provide occupational therapy and physiotherapy to people are best treated in their own home. Our team undertake assessments of people, in relation to their activities of daily living, and support them to rehabilitate and achieve an optimal level of functional independence.
They do this by helping people (and their carers) to adapt their techniques e.g. the way they do things, learn how to self-manage, carry out exercise programmes, and access/use adaptive equipment.
They also work in partnership with other community-based services in Wirral and can refer people onto other services that will be able to meet their ongoing needs within the community.
How to access our service?
Your GP or any other health or social care professional involved in your care can refer you to our Rehabilitation at Home Service. If you meet the criteria of the service, the team will arrange to see you at home.
You may be referred on elsewhere to another service or agency if this is appropriate for you.
What happens at your first appointment?
At your first appointment one of our occupational therapists will find out what your needs are and work out a way to help you achieve your goals.
We will put you in touch with others who may be able to help and arrange to see you again if you need further occupational therapy and physiotherapy in your home environment.
