Transfer of service
From Monday 1 May the delivery of Wirral’s Telehealth Service will transfer to Mersey Care NHS Foundation Trust, the primary provider for Telehealth across Cheshire and Merseyside.
All current patients will be contacted and should not experience any disruption to care as a direct result of this transfer.
Patient care records will transfer to new provider and will remain secure and confidential in line with Mersey Care’s privacy statement.
About the service
The Telehealth Service monitors and supports people with long-term conditions such as heart failure and chronic obstructive pulmonary disease (COPD). The remote monitoring service also provides support for people who have tested positive for Covid-19.
Telehealth simply means providing health care at a distance rather than face-to-face.
Our team of highly trained and experienced nurses provide dedicated help, support, and education to people living with heart failure and COPD in Wirral.
The Telehealth team helps people to:
- Learn more about their condition
- Monitor it at home with dedicated equipment and support
- Manage their condition carefully to avoid periods of illness and unnecessary hospitals visits and admissions
With the use of technology and specialist equipment, such as blood pressure machines, pulse oximeters, thermometers and scales people are empowered to self-manage their condition effectively at home. By understanding their symptoms and triggers and knowing their numbers along with support from our dedicated clinical team, people can recognise when their condition is deteriorating and access the medical help they need.
What to expect
The service operates on a referral basis only.
Once you have been referred into the Telehealth Service you will receive telephone call from one of our nurses to discuss the support available and arrange a mutually convenient time for your equipment to be delivered to your home by the Docobo team.
Following this a member of the Docobo team will visit your home to install the equipment and show you how to use it. Your equipment will also include a handheld device which you will use regularly to record your readings e.g. blood pressure or oxygen saturations reading. Your device is connected to the secure monitoring hub within the Telehealth Service.
Once you are set up will need to submit your readings regularly – your nurse will advise you on the frequency. All submitted readings will be monitored regularly by the team. If there are any concerns a nurse will contact you.
How to access the service
Anyone over the age of 18 years with heart failure, chronic obstructive pulmonary disease (COPD) or Covid-19 can access the service via a referral from their GP or health and care professional.
Referrals can only be accepted from health and care professionals across Wirral e.g. GP, community nurses, heart failure nurses, allied health professionals (AHPs) or community matrons.
The service is hoping to accept referrals for other long-term conditions in the future.
