Discharge and Liaison

The Discharge and Liaison Team support the effective, safe and timely transfer of patients who require community nursing care after they leave hospital.

The team triage nursing assessments for patients who:

• have ongoing health and social care needs
• require community healthcare services following discharge from hospital.

Every patient referred to the service is triaged to identify what ongoing care is required on discharge (including ordering any equipment required) in order to make the discharge process as smooth as possible.

The team works closely with other professionals involved in the care of the patient, including allied health professionals, specialist nurses and social workers.
