We will ask for your name and date of birth.
If the patient is a child we will ask if you are responsible for them.
Tell us if you have any medical problems and if you are taking any medicine.
We will ask about your dental problem.
At the end of the call we will ask how well your problem was dealt with.
If you need treatment we will call you to talk about the cost.
If you do not pay for treatment, you must have proof of this.
