Dear parents,
Now that your child is about to start school, we would like to ensure that you have the details of our 0-19+ Contact Hub as we plan the transition of your child’s care from the Health Visiting to the School Nurse Service (some specialist settings may have their own school nurse service).
As we are a 0-19+ Service any support that you require for your child in the future can be accessed via our main Contact Hub on 0300 123 4058 or via our website – www.wchc.nhs.uk/children. The health visiting service will continue to support you and your child with any outstanding health needs and transition to school for the first term – you can contact the team via the Contact Hub.
