Who we are / a bit about what we do
Teletriage is an award winning service, operating 24 hours a day, 365 days a year.
Our team is made up of senior healthcare professionals who provide healthcare advice and support to care home staff across Wirral to help manage any health concerns they have about their residents. They also facilitate virtual and rapid clinical assessments.
Care homes no longer need to use the NHS 111 service for non-emergencies. If a care home is concerned about a resident they contact our service. Using Skype on iPads the senior nurse carries out a virtual and rapid clinical assessment of the patient to determine the care/treatment required. Following this senior nurses coordinate the appropriate care for the resident ideally in their own care home.
Care/treatment may include:
- advice and support to care home staff
- referrals to other services such as local GPs, relevant community services ie community nurses, the falls team, geriatrician (healthcare professional who specialises in the health and care of older adults)
Working in this way residents are given the care and treatment they require in their own familiar and comfortable environment, rather than facing the frightening and disruptive
prospect of ambulances and hospital departments.
If an emergency ambulance is needed our team will arrange this.

Our team offers support to care home staff and provides care and treatment for patients with non-urgent medical conditions in care homes across Wirral.

Additional self-care information
Our team ensures all care homes are kept up-to-date with healthcare advice. Care home staff also receive direct support and on-going clinical supervision.
