The Care Arranging Team (CAT) is based at St Catherine’s Health Centre and works across the Trust and wider services supporting, Wirral Borough Council and Community Mental Health Teams in commissioning care and procuring care services.
The team ensure that care services are commissioned in a timely way and from the appropriate contracted providers. They manage the procurement and brokering of care and support packages including: respite care, residential and nursing placements and supported living accommodation and care for residents living in Wirral.
Referral Process
Referrals are received from social work teams, hospital teams, Continuing Health Care, Crisis Response and Mental Health Teams.
All of the above can make a referral through in-house system Liquid Logic. Once a referral is made, the team source the most appropriate package for the client to meet their specific care needs.
